Member Health Rewards
Maryland Physicians Care members who have completed a certain prevention and wellness health event could earn a healthy rewards. It’s easy!
Incentives are available only for eligible services completed in 2026. Your request and proof of service, such as a provider bill or claim, must be received by March 1, 2027.
Breast Cancer
Screening:
If you are a woman age 42–64 and have not had a mammogram in 2025 or 2026, you may be eligible for a $75 gift card. Complete your mammogram between September 15 and December 31, 2026, then complete the registration.
After your appointment complete the Gift Card registration for a $75.00 gift card.
Colon Cancer Screening:
If you are age 45 or older and have not completed a colon cancer screening in 2026, you may be eligible for a $75 gift card. Talk to your doctor about which screening is right for you. Complete your screening between September 15 and December 31, 2026, then complete the registration
After your appointment complete the Gift Card registration for a $75.00 gift card.
Diabetes and A1c Testing:
If you have diabetes and have not had an A1c test in 2026, you may be eligible for a $50 gift card. Complete your A1c blood test between September 15 and December 31, 2026, then complete the registration.

Diabetic Eye Exam
If you have diabetes and have not had a diabetic eye exam in 2026, you may be eligible for a $75 gift card. Complete your diabetic eye exam between September 15 and December 31, 2026, then complete the registration.

Postpartum:
Congratulations on the birth of your new baby. Don’t forget to take care of you too mom. To reward your good health, MPC will provide mom’s who complete their postpartum check up with a $50.00 gift card. Your checkup must occur within 7 to 84 days after you have your baby.

Well Child Visit:
Maryland Physicians Care provides services that help children grow up healthy and happy. It is important for infants, children, and adolescents up to age 21 to receive regular checkups. Regular and routine wellness visits help your provider identify, treat, and prevent health problems. Wellness visits are comprehensive and include medically necessary medical, vision, and hearing assessments and services
After your appointment, complete the registration to enter into a quarterly raffle.
