Find A Provider
Instructions: With our simple finder tool, you can search for a
doctor or hospital three different ways.
Search by name. Enter the last name of the doctor or the name of
the hospital you want.
Search by location. Enter a ZIP code. To search in a larger area,
enter the ZIP code and select how many miles around it you would like to search.
You can also search by city.
Search by specialty. Use the drop-down box to find a doctor by
specialty. For eye care, click here.
Please remember:
- HealthChoice members do not need a referral from their primary care physician (PCP)
to visit most specialists.
- Members need to call Maryland Physicians Care Member Services at 1-800-953-8854
before seeing a new PCP.

to speak with someone who will help you find a doctor to answer your eye care questions.
Dental Care
For questions about your adult dental coverage (age 21 and older), please call DentaQuest

call Healthy Smiles Dental Services toll free at 1-888-696-9596.
